Generic Name: Zongertinib
Brand Name: LuciZonger
Drug Class: Selective HER2 tyrosine kinase inhibitor (TKI)
Dosage Form: Oral film-coated tablet
Strength: 60 mg
Route of Administration: Oral
Manufacturer: Lucius Pharmaceuticals

Indication:

LuciZonger is a targeted therapy indicated for adult patients with non-small cell lung cancer (NSCLC) harboring activating HER2 (ERBB2) mutations, particularly mutations in the tyrosine kinase domain, as confirmed by an approved diagnostic test.

Mechanism of Action:

Zongertinib works by selectively and irreversibly inhibiting the HER2 tyrosine kinase domain, blocking abnormal HER2-driven signaling pathways (including PI3K/AKT and MAPK). This suppresses tumor cell growth and promotes cancer cell death while largely sparing wild-type EGFR, which may reduce certain off-target toxicities.

Recommended Dosing:

  • Typically 60–120 mg orally once daily, as prescribed
  • Taken with or without food
  • Tablets should be swallowed whole with water
  • Dose adjustments may be required based on tolerability and adverse reactions

Common Adverse Reactions:

  • Diarrhea
  • Skin rash
  • Fatigue
  • Nausea
  • Decreased appetite

Serious or Less Common Risks:

  • Elevated liver enzymes
  • Cardiac effects (QT prolongation, reduced cardiac function)
  • Interstitial lung disease / pneumonitis (rare)
  • Electrolyte disturbances

Monitoring & Precautions:

  • Regular monitoring of liver function, electrolytes, and cardiac status
  • Not recommended during pregnancy or breastfeeding
  • Safety and efficacy in pediatric patients have not been established
  • Use under specialist oncology supervision only

Clinical Significance:

LuciZonger represents a next-generation, precision-designed HER2 inhibitor with demonstrated activity in HER2-mutant NSCLC, including patients with brain metastases. Its selectivity for HER2 supports effective tumor control with a potentially improved tolerability profile compared with less selective TKIs.
